Dr. Crow does not participate in any insurance plans and is considered an Out-Of-Network Provider. Payment is due at the time of service. If you are interested in submitting claims for reimbursement, it is recommended that you first call your insurance provider prior to your first appointment to determine what type, if any, reimbursement can be expected. Clients will be provided with detailed Statements for Insurance Reimbursement with the information needed to submit your claims.
It is recommended that you call the number on the back of your insurance card to inquire about Out-Of-Network benefits. Please note that some insurance providers have a separate phone number for mental/behavioral health benefits. The following are recommended questions to ask your insurance company:
Do I have Out-Of-Network coverage for mental health services (see CPT codes below)?
Do I need a referral for services?
What is my deductible? Has it been met?
Once I meet my deductible, what is my copay/coinsurance?
Is there a limit to the number of sessions covered per year?
How do I submit for insurance reimbursement?
